Firefox plugins (eg flash plugin) may store preferences for a particular site permanently. To reset such stored permissions use:
- cd ~/.mozilla
- cat profiles.ini
- cd <default-profile-folder>
- sqlite3 permissions.sqlite
- select * from moz_hosts where lower(host) like '%<site>%';
- delete from moz_hosts where id=<first-col-value>;
- .quit
